Rochdale Council’s Rochdale Adult Care is offering an opportunity for an Assessment and Support Planner to join the Home First Hospital Discharge Hub at Brook House Offices, Middleton, Greater Manchester.

The post holder will be a dynamic team player with strong written and verbal communication, able to work to deadlines with adults, carers and hospital professionals, and willing to learn within a supportive service.

Qualifications

• Knowledge or experience in Adult Care settings.
• Experience with strength-based assessments and support planning.
• Promoting independence and well-being per Care Act 2014.

Responsibilities

• Work with adults, carers and hospital professionals within timescales and deadlines.
• Travel around the Borough of Rochdale and hospital sites.
• Support the Home First Hospital Discharge Hub and related services.

Rochdale Adult Care is pleased to offer an opportunity for an Assessment and Support Planner to join Adult Care Services within the Home First Hospital Discharge Hub based within the Brook House Offices in Middleton. It is an excellent opportunity to join a vibrant and creative authority.

The post holder will be expected to be a Team Player, have excellent written and communication skills and a willingness to learn. The post holder will be expected to be a dynamic fast paced individual, who holds the appropriate skills, experience and motivation to work with Adults, their Carers and hospital professionals, whilst working within timescales and deadlines to produce a high quality of work. In return you will have the opportunity to further develop your skills within a supportive and enthusiastic service.

It is expected that the post holders will have some knowledge or experience of working within an Adult Care setting, strength based approaches to assessments and support planning and supporting service users and carers, promoting independence and well-being, as within the objectives of the Care Act 2014.

This post is ideal for anyone with an interest in working with adults to gain practical experience within a supportive Adult Care Setting. There is an opportunity to undertake a variety of appropriate training and development courses.

The Teams within the Integrated Neighbourhood Teams work across the following Localities and cover a wide range of work with Adults over 18.

• Heywood


• Middleton & Home First Hospital Discharge Hub


• Pennine


• Prevention Front Door Service


• Rochdale North


• Rochdale South

The post holder’s must be able to demonstrate an ability to travel around the Borough of Rochdale and the Hospital Sites out of area. The post is subject to a seven day working contract on a rota basis.

Appointment to this post is subject to an enhanced DBS including a barred list check adult workforce
