About the role

To provide an assessment, support planning, monitoring and reviewing function for people and / or carers. To work as part of a multi-disciplinary team and to undertake core functions as required. Support people to identify their own individual needs and desired outcomes and explore and support them to improve their well-being. To carry out activities under the direction of an Advanced practitioner Team Manager, or other professional staff.

Working in a busy, supportive Intermediate Care Social Work team, we undertake assessments and support planning for those individuals who are either leaving the Intermediate Care Unit or have had a period of reablement with our Home First service. Working closely as part of an MDT, we value the close working relationships with other professionals which all contribute towards successful outcomes for our residents.

About you

We are looking for an individual who possesses a positive attitude, who is open to change and can empower others. A person who has integrity and honesty and treats people fairly, champions and appreciates diversity. A person who has the ability to actively listen and remain person centred in all interactions. You must be a good communicator, and possess the ability to deliver sensitive information in a compassionate manner, particularly as you will be working with individuals/families who are experiencing change and may be feeling particularly vulnerable. Ideally you would have experience of working in Adult Social Care and be familiar with legislation and guidance that underpins our work.

Please be aware that this post is subject to a Disclosure and Barring Service (DBS) check and that you will be required to meet the cost of the DBS check currently £49.50. The payment for this will be deducted from your first four months’ salary payments at a rate of £12.38 per month. If you do not commence employment for any reason but the DBS has been processed, you will be sent an invoice for the payment of £49.50.
