Job summary

Rochdale Adult Care is offering a permanent Assessment and Support Planning Officer role within the South Integrated Neighbourhood Team. You will work with adults, families and carers to identify needs, complete strengths-based assessments, reviews and support planning to promote independence and wellbeing.

The post involves collaboration with health partners and community organisations, a busy multidisciplinary team, and a requirement to travel across the borough as needed. DBS check applies.

Qualifications

• Experience with adults, families, carers or vulnerable people.
• Strong assessment, problem-solving and organisational skills.
• Ability to manage a varied workload and work to timescales.
• Person-centred and strengths-based approach.
• Confidence working with a range of professionals and agencies.
• Good IT and recording skills.

Responsibilities

• Carry out strengths-based assessments and reviews.
• Develop and implement support plans to promote independence and wellbeing.
• Support people to achieve outcomes in line with Care Act 2014.
• Collaborate with health partners, providers and community organisations.
• Willingness to travel across the borough as required.
