Job summary

Rochdale Council's Adult Care service invites applications for a permanent Assessment and Support Planning Officer within the South Integrated Neighbourhood Team. You will work with adults, families and carers to identify strengths, assess needs and support people to achieve outcomes that matter most, completing strengths-based assessments, reviews and plans.

Qualifications

• Experience of working with adults, families, carers or vulnerable people.
• Strong assessment, problem-solving and organisational skills.
• Ability to manage a varied workload and meet timescales.
• A person-centred and strengths-based approach.
• Confidence working with a range of professionals and agencies.
• Good IT and recording skills.
• Experience in Adult Social Care is desirable; transferable skills from health, housing, voluntary, support, community or related services accepted.
• Willingness to travel across the borough is essential.

Responsibilities

• Identify strengths and assess needs of adults, families and carers.
• Complete strengths-based assessments, reviews and support planning.
• Promote independence, wellbeing and choice in line with the Care Act 2014.
• Collaborate with health partners, providers and community organisations.
