Job summary

Rotherham Metropolitan Borough Council is seeking three Assessment & Review Coordinators on a full-time basis (37 hours per week) based at Riverside House, Rotherham. The roles involve working within Adult Social Care Locality Teams to assess and review needs, safeguarding adults, and coordinating services.

  • Knowledge of statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, Making Safeguarding Personal, and Continuing Healthcare Frameworks.
  • Background in Housing, Welfare & Benefits, Mental Health or Education is welcome.
  • Ability to work in varying complexity and make timely decisions to support people in crisis.

Responsibilities

  • Assist with assessments, care planning, and safeguarding for adults.
  • Collaborate with Locality Teams across North, Central and South areas.
  • Support carers and families to maximise independence and resilience.
  • Liaise with partner agencies to identify solutions and enable positive risk-taking.

Our Locality Teams work with adults of all ages, who are experiencing the impacts of short- or long-term health conditions, physical and/or cognitive impairments. We work holistically to support carers and families, valuing the support they provide too.

  • Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, Making Safeguarding Personal, and Continuing Healthcare Frameworks.
