Ensure seamless patient care by collaborating with hospital teams, social services, and community providers.

Patient-Centred Care:

Treat every patient with respect, kindness, and confidentiality.

Guide patients to the most appropriate services or support within the Primary Care Network (PCN).

Main Duties of the Job:

• Act as a key link between GPs, clinicians, external agencies, and patients.
• Collaborate with hospital teams, social services, and community providers to ensure seamless care.
• Provide patient-centred care by respecting confidentiality and guiding patients appropriately.

Job Responsibilities:

• Act as a key link between GPs, clinicians, external agencies, and patients.

Person Specification:

Qualifications:

• NVQ Level 2 or 3 in Health and Social Care or equivalent.
• Experience supporting individuals, families, and carers.
• Good IT skills and understanding of IT systems.
• Excellent communication, record-keeping, and time management skills.

Desirable:

• Experience in data collection.
• Knowledge of policies on confidentiality, safeguarding, information governance, health and safety.
