Qualifications

• Demonstrable experience in the role of a Recovery Worker, Treatment/Rehab Practitioner or Outreach Worker within the community or hostel setting.

• Experience working with individuals in recovery from alcohol/substance misuse.
• Training and knowledge of harm reduction practices to educate people on reducing risk.

Responsibilities

• Work as part of a team supporting New Start's aftercare and relapse prevention services.
• Focus aftercare on recovery, tenancy sustainment, community reintegration and independent living skills.
• Ensure cases are involved in education, employment, training and meaningful activities.
• Target assistance to those struggling with recovery per personal relapse prevention plans.
• Provide crisis support and implement safety and support plans.
• Assist tenancy stability and housing options for residents in Liverpool.
• Adhere to Health and Safety regulations and support team in this area.
• Complete administrative tasks and maintain accurate case notes.

An exciting opportunity has arisen for a experienced and enthusiasticAFTERCARE & RELAPSE PREVENTION PRACTITIONERwithin New Start's Adult Services division. The successful candidate will support the Step-down Service and the Harmony Project offering advice, guidance and targeted support to residents working on their personal recovery journey.

New Startis a family-run, not-for-profit organisation, providing residential and supported accommodation services for vulnerable adults, families, children and young people. We deliver a diverse range of homelessness projects and temporary housing solutions, helping some of the most complex and disadvantaged groups across Merseyside. Responsibilities of our Aftercare & Relapse Prevention Practitioner:

• To work as part of a team that supports New Start's aftercare offer, on-site abstinence offer and dispersed services abstinence provision.
• Focusing aftercare support around recovery and tenancy sustainment, community reintegration, independent living skills and resettlement
• Ensuring aftercare cases are involved in ETE, meaningful use of time, have access to opportunities, and are supported to build social capital to strengthen their recovery and keep positively on track
• Targeting assistance to those struggling with their recovery in-line with their personal relapse prevention plan
• Providing crisis support to individuals whose recovery breaks down, ensuring a safety management plan is put in place and the adequate support required is devised and implemented
• Targeting support to those struggling with their tenancy, taking a solution-focused and preventative approach that will help keep them housed
• Abiding by Health and Safety regulations and supporting others in this area
• Undertaking operational, administrative and recording tasks required for the role

What are we're looking for in a Aftercare & Relapse Prevention Practitioner:

• Demonstrable experience in the role of a Recovery Worker, Treatment/Rehab Practitioner or Outreach Worker within the community or within a hostel setting
• Demonstrable experience of handling crisis situations, remaining calm and focused whilst the problem(s) is addressed
• Experience and knowledge of working with individuals in recovery from alcohol/substance misuse and the challenges faced by those on a journey of recovery striving to remain abstinent
• Experience, training and knowledge of harm reduction practices and strategies to educate people on how to reduce their risk of harm in substance misuse cases
• NVQ Level 3 in Housing/Health and Social Care/or equivalent qualifications
• The ability to accurately record information within case notes and support plans professionally and effectively
• Experience supporting vulnerable individuals through crisis or periods of change
• Experience and knowledge of how to access the Property Pool Plus System and alternative housing options for residents within the city of Liverpool
• Knowledge of opportunities, community facilities and ETE providers across the city
• Experience of working with adults with multiple and complex needs who can exhibit challenging behaviour
• Excellent understanding of professional boundaries when working with vulnerable adults
• Experience of completing risk assessments and incident reports to a high standard
• Current good working demographical knowledge of the city of Liverpool
• A flexible approach to ensure high level support to caseload
