Job summary

Qualifications

• Essential: recognised professional qualification in social work and registration with Social Work England.
• Significant experience in adult social care and understanding of relevant acts and safeguarding.
• Experience with Continued Health Care (CHC) processes and checklists.

Responsibilities

• Manage caseloads of adults with care and support needs, helping identify goals and practical solutions.
• Complete high-quality reviews including needs assessments, care and support planning and safeguarding enquiries.
• Promote dignity, choice and wellbeing while balancing safety and independence.
• Collaborate with colleagues across functions and partner organisations.
• Maintain accurate, confidential records in line with standards.
