Job summary

The NHS Jobs Advanced Pharmacist Practitioner role focuses on medicines optimisation, prescribing and chronic disease management within a primary care setting. You will conduct medicines reviews, support safety and cost-efficiency, and contribute to quality governance across the practice.

The post requires GPhC registration, independent prescriber status, and significant primary care experience. Excellent communication, organisation, and ability to work independently within a multidisciplinary

Qualifications

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Independent Prescriber qualification (or working towards obtaining one).
• Experience in Primary Care, General Practice, PCN, or Community Pharmacy.
• Strong knowledge of medicines optimisation, prescribing, and long-term condition management.
• Excellent communication, consultation, and organisational skills.
• Ability to manage workload independently while working collaboratively within a multidisciplinary team.

Responsibilities

• Medication reviews.
• Medication safety alerts and medicines optimisation.
• Supporting CQC compliance and quality improvement.
• Chronic disease management (Diabetes, Hypertension, Asthma and COPD as required).
• Prescriptions and repeat prescribing management.
• Blood test monitoring and management, including patient telephone consultations.
• Minor ailments assessment and management.
• Supporting safe and effective patient care across the practice.
