Job summary

Qualifications

Advanced Pharmacist Practitioner / Advanced Clinical Pharmacist

Key ResponsibilitiesMedicines Optimisation and Prescribing Conduct face-to-face and remote structured medication reviews. Support safe, effective, and cost-efficient prescribing. Process and manage prescription requests and medication queries. Review and implement medication safety alerts, MHRA guidance, and prescribing updates. Undertake medicines reconciliation following hospital discharge and clinic correspondence. Manage repeat prescribing systems to ensure safe and appropriate medication use. Chronic Disease Management Provide clinical support and patient reviews for long-term conditions, including: Diabetes Hypertension Asthma COPD This may include medication optimisation, treatment escalation, patient education, monitoring, and adherence support as required. Blood Test and Medication Monitoring Review blood test results related to medication monitoring. Contact patients by telephone regarding abnormal or outstanding results. Arrange and monitor required follow-up investigations in line with clinical protocols. Ensure compliance with medication monitoring requirements and clinical safety standards. Minor Ailments Service Assess, diagnose, and manage patients presenting with minor illnesses and common ailments within your scope of practice. Provide treatment advice, prescribing where appropriate, and onward referrals when necessary. Quality, Safety and Governance Support the practice in achieving and maintaining CQC compliance and quality standards. Assist with clinical audits, quality improvement projects, and medicines management initiatives. Identify and mitigate prescribing risks to improve patient safety. Contribute to practice policies, protocols, and medicines-related governance activities. Essential Requirements Registered Pharmacist with the General Pharmaceutical Council (GPhC).
