Job summary

We are looking for an enthusiastic and committed Advanced Nurse Practitioner to join our friendly and supportive team.

We pride ourselves on providing high-quality, patient-centred care while supporting our clinicians. You will work alongside an experienced team of GPs, ANPs, practice nurse, pharmacists, dispensers, healthcare professionals and an excellent admin support team. The role is aimed at an ANP wanting to develop their breadth of skills and clinical knowledge beyond the management of acute minor illness.

Main duties of the job

You will provide autonomous clinical assessment, diagnosis, treatment and ongoing management of patients presenting with a broad range of acute and long-term conditions. Working closely with our GPs and wider multidisciplinary team, you will deliver safe, effective and holistic patient care. The role offers the opportunity to work alongside our experienced GPs to develop and expand the breadth of your clinical skills and knowledge. This will be achieved by daily professional discussion with the GP, managing ones own blood results (training and support given) re-booking follow up appointments with yourself to promote continuity of care. You will be mentored to have confidence in taking on a varied caseload beyond the management of acute same day minor illnesses.

Hodnet Medical Centre is a rural dispensing practice with a list size of 4200. Situated in the historic village of Hodnet, North Shropshire, 15 miles north of Shrewsbury. With good transport links to Birmingham and Manchester.

We have traditional values. Being a small practice with a stable patient population we get to know our patients, often many generations of the same family. We operate from a modern building extended from an original grade two listed building which adds to the character and charm of our Practice.

Job responsibilities
  • Works autonomously and accountable for own professional actions.
  • Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
  • Assess, diagnose and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
  • Assess, diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.
  • Admit patients to secondary care hospital in acute medical need.
  • Refer patients to other medical specialities for assessment if deemed appropriate.
  • Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.
  • Interpret a range of diagnostic tests and routine clinical procedures.
  • Make decisions where precedents do not exist, where appropriate without recourse to others e.g. direct referral of patients to Hospital Consultants without prior reference to other medical colleagues.
  • Advise and support others where standard protocols do not apply.
  • Work within professional guidelines and codes of conduct.
  • Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
  • Extended and supplementary prescriber within scope of expertise
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.

  • Record health data correctly to meet QOF and enhanced service specifications
  • Support and participate in shared learning and clinical meetings across the practice
