Lister House Surgery in Derby is seeking an experienced Practice Nurse to join our friendly nursing team. The role offers up to full-time hours with opportunities to mentor others and develop specialist interests within the practice.

Responsibilities

Job Summary

We are seeking an experienced and enthusiastic Practice Nurse to join our friendly nursing team for up to full time hours.You will be committed to providing excellent patient care, working collaboratively with the wider clinical team, and contributing to the ongoing development of services within the practice.This is an excellent opportunity for a motivated nurse looking to take on a varied role with opportunities to support and mentor others, develop specialist interests, and play an active part in the future of our practice.

Main duties of the job
  • Deliver high-quality nursing care to patients in a general practice setting.
  • Undertake cervical screening, vaccinations and immunisations, including seasonal flu clinics.
  • Conduct long-term condition reviews and medication reviews within scope of practice and qualifications and clinical protocols.
  • Provide wound care management, including dressing, sutures, and clip removal.
  • Perform a range of clinical procedures including ECGs, ear irrigation, injections, and phlebotomy-related tasks where required.
  • Deliver travel health advice, vaccinations and contraception services within scope of practice.
  • Process laboratory samples and maintain accurate, timely, and confidential patient records.
  • Ensure safe management, storage, and monitoring of vaccines, medicines, and cold chain processes.
  • Work collaboratively within the multidisciplinary team, making referrals and supporting patients to access appropriate services.
  • Maintain professional standards in line with the NMC requirements, participate in training and audit activities, and support continuous quality improvement within the practice.
Job Description/Responsibilities
  • Long term condition reviews (according to qualifications)
  • Medications reviews, providing information on prescribed or over the counter medication
  • General and hormonal injections
  • Child and adult vaccinations and immunisations (as agreed with the Practice)
  • Contraceptive pill checks and follow up contraceptive injections (over 18s) unless has specialist qualifications / training for under 18's.
  • Ear examinations and syringing
  • ECG recordings
  • Travel health
  • Wound care management and suture/clip removal
  • Chaperoning
  • Referrals to extend hours clinicals/external agencies
  • Processing and management of laboratory samples
  • Vaccine/cold chain storage and monitoring
  • Flu vaccinations
  • General treatment services may be required within scope of practice
  • As directed by the Practice provide private nursing services which fall outside of the NHS and support the collection of private fees for such work
  • Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)
  • Maintain Practice Indemnity cover is kept up to date by responding to any communications
  • Attend monthly clinical meetings, monthly nurse meetings and relevant team meeting sessions in order to support and participate in shared learning
  • Produce accurate, contemporaneous and complete records of patient consultations, consists with legislation, professional regulations and Practice policies and procedures
  • Initiate and change or recommend changes if a non-medical prescriber medicine regimes as required, in line with medicine management guidelines, local and national formularies
  • Initiate appropriate investigations, interpretations of results and timely actions
  • All records must be coded appropriately on the clinical computer system in accordance with QoF, enhanced services and Practice policies ad protocols
  • Deliver care according to current guidelines including the National Institute for Clinical Excellence (NICE) guidelines, Chief Medical Officer guidance and evidence-based care
  • Assess effectiveness of care through self and peer review, benchmarking and formal evaluation
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
  • Participate in the management, review and identification of learning from patient complaints, clinical incidents (significant events) and near-miss events
  • Understand and apply legal policy hat supports the identification of vulnerable and abused children and adults, being aware of child/vulnerable adult health procedures and local guidance
  • Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour and refer as appropriate
  • Ensure personal hygiene, image and dress code is appropriate to reflect good infection control procedures, high professional standards and an efficient and organised service
