Job summary

We are looking for an experienced and enthusiastic Advanced Nurse Practitioner (ANP) to join our friendly, supportive and forward-thinking team for three days per week. We can offer flexibility around working days; however, Wednesday is an essential working day for this role. This is a varied and rewarding position, combining care home and community-based work with a face-to-face minor illness clinic at the practice. The role offers the opportunity to work autonomously while remaining an integral part of our wider multidisciplinary clinical team. The successful candidate will be an independent prescriber with the confidence, clinical knowledge and experience to independently assess, diagnose and manage patients presenting with a wide range of acute and minor illness presentations. We are looking for someone who is comfortable working autonomously, demonstrates sound clinical decision-making and enjoys working collaboratively with GPs, nurses and the wider practice and PCN team to provide high-quality patient care.

Main duties of the job

This is a varied and autonomous ANP role combining care home, community and practice-based clinical work. The successful candidate will work closely with our Care Homes Lead to support residents at Hurstway Care Home, providing proactive and responsive clinical care, as well as undertaking home visits for other practice patients where required. The ANP will run a face-to-face acute and minor illness clinic at the practice, independently assessing, diagnosing and managing patients with a range of presentations. As an independent prescriber, you will use your clinical judgement to assess and manage patients, prescribe where appropriate, arrange investigations and referrals, and provide appropriate safety-netting and follow-up. You will work autonomously while being supported by and working collaboratively with our GPs, nursing team and wider multidisciplinary team.

You will join a supportive multidisciplinary team including GPs, Advanced Practitioners, nursing staff, healthcare assistants and additional roles working across Primary Care. We are keen to develop services that allow clinicians to work proactively with our more vulnerable patients while ensuring patients can access appropriate same-day and routine care within the practice. The postholder will also undertake home visits to other practice patients where clinically required and work collaboratively with GPs, care home staff and the wider multidisciplinary team.

Essential

  • Registered Nurse with current NMC registration.
  • Advanced clinical practice qualification at Masters level or equivalent.
  • Independent Prescribing qualification (V300).
  • Evidence of relevant continuing professional development.

Desirable

  • Qualification or additional training in advanced clinical assessment.
  • Relevant training in the management of minor illness and/or long-term conditions.

Experience

Essential

  • Experience working as an Advanced Nurse Practitioner or in a comparable autonomous advanced clinical role.
  • Experience independently assessing, diagnosing and managing patients with acute and minor illness.
  • Experience of autonomous clinical decision-making and managing clinical risk.
  • Experience of independent prescribing.
  • Experience working collaboratively within a multidisciplinary team.

Desirable

  • Previous experience within General Practice or Primary Care.
  • Experience of caring for older people and patients with complex health needs.
  • Experience of care home and/or community-based clinical work.
  • Experience undertaking home visits.
  • Experience using EMIS Web.
