Job summary

NHS Forth Valley is seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience working within Care Homes or Community Hospitals, management of patients with high clinical frailty scores and undifferentiated complex care presentations.

Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions and to lead sensitive discussions

Qualifications

• Must be PVG scheme member
• Band 7 requirements with prescribing qualifications
• Experience in advanced practice desirable

Responsibilities

• Assess and treat frail, older patients presenting with acute deterioration in care homes or community hospitals
• Lead sensitive discussions around anticipatory care planning and end-of-life care
• Work across organisational boundaries with health and social care teams
