An exciting opportunity has arisen for motivated and enthusiastic Advanced Frailty Practitioners to join the Home First Response Service (HFRS) within NHS Greater Glasgow and Clyde.
We are seeking clinicians with a passion for caring for older people and supporting the delivery of person, centred, frailty-focused care across the acute and community interface. The Home First Response Service works closely with Emergency Departments, Acute Assessment Areas, and Hospital at Home, community teams and wider multidisciplinary partners to prevent unnecessary hospital admissions, facilitate early discharge, and support patients to remain at home wherever possible.
The service plays a key role in improving outcomes for frail older adults through comprehensive assessment, clinical decision-making, care planning and coordination of services across health and social care settings.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Please note the salary is Pro Rata for part time hours.
Applicants should be registered healthcare professionals (e.g. Nurse, Allied Health Professional or Paramedic) with substantial post-registration experience, ideally within older people's services, urgent care, emergency care, acute medicine, Hospital at Home or community services.
