Job summary

NHS Greater Glasgow and Clyde is inviting applications for an Advanced Frailty Practitioner to join the Home First Response Service (HFRS) at Queen Elizabeth University Hospital. This Band 7 role involves working across acute and community interfaces to prevent admissions and support early discharge.

We seek clinicians with passion for older people, with post-registration experience in frailty or urgent care, and postgraduate qualifications in advanced clinical practice or clinical assessment.

Qualifications

• Applicants should be registered healthcare professionals with substantial post-registration experience, ideally within older people\'s services, urgent care, emergency care, acute medicine, Hospital at Home or community services.
• Relevant postgraduate qualifications in either advanced clinical practice, frailty, or clinical assessment would be required.

Responsibilities

• Undertaking comprehensive frailty assessments for older adults.
• Supporting clinical decision-making and development of management plans.
• Facilitating safe discharge and admission avoidance where appropriate.
• Working collaboratively with acute, community and social care partners.
• Coordinating referrals and onward care arrangements.
• Providing leadership and expert clinical advice within the multidisciplinary team.
• Supporting service development and quality improvement initiatives.
