Conexus Healthcare C.I.C., Wakefield, invites applications for an Advanced Nurse Practitioner to join a newly established Integrated Neighbourhood Care Team aimed at transforming care for housebound patients and residents of care homes. The post holder will work autonomously and collaboratively to deliver high-quality clinical care, leadership, and service development.

The role focuses on proactive population health management, urgent and reactive care, end of life care, and building capability

Competenze

• Essential: Experience of working in a primary care setting.
• Essential: Post graduate degree in ANP or ACP and independent prescribing.
• Essential: Registered Healthcare Practitioner.
• Desirable: Teaching/mentorship qualification.
• Desirable: Experience with care homes and palliative care; knowledge of SystmOne/EMIS.

Mansioni

• Identify and manage patients at risk of deterioration, hospital admission or increasing frailty.
• Undertake comprehensive holistic assessments for housebound patients and care home residents.
• Develop, implement and review personalised care and support plans.
• Lead case management of patients with complex long-term conditions, frailty and multimorbidity.
• Prescribe safely and effectively within scope of practice using independent prescribing qualifications.
• Liaise closely with ambulance services, community services and secondary care colleagues.
