Job summary

The Village Surgeries, Croston and Eccleston are seeking an Advanced Nurse Practitioner to join our well-established GP practices across two sites. You will diagnose, manage, prescribe and discharge patients across all ages, with opportunities for face-to-face and telephone consultations.

Qualifications

• Autonomous clinical assessment and treatment experience in a GP setting.
• Strong clinical decision-making and triage abilities.
• Excellent communication with patients and colleagues.
• Experience in General Practice and patient safety.
• Proven ability to lead service improvements or audits.

Responsibilities

• Assess, diagnose and manage patients with undifferentiated and acute conditions.
• Conduct clinical examinations and request/interpret investigations.
• Develop and implement treatment and management plans.
• Prescribe safely within scope and prescribing qualifications.
• Provide same-day and urgent care when needed.
• Recognise acutely unwell patients and escalate appropriately.
• Refer and provide safety-netting advice.
• Maintain accurate, timely clinical records.
• Collaborate with GPs and the wider multidisciplinary team.
• Contribute to clinical governance, audits and service development.
• Participate in clinical meetings and CPD.
• Complete home visits.

Advanced Nurse Practitioner (ANP)

Main duties of the job

The successfulAdvanced Nurse Practitionerwill work as an autonomous practitioner, diagnosing, managing, prescribing, and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, or telephone.

Job responsibilities

• Assess, diagnose and manage patients presenting with undifferentiated and acute conditions.
• Undertake clinical examinations and request and interpret appropriate investigations.
• Develop and implement appropriate treatment and management plans.
• Prescribe safely within your scope of practice and prescribing qualification.

Person Specification

Experience

• Demonstrable experience of autonomous clinical assessment, diagnosis and treatment.
• Excellent communication and clinical decision-making skills.
• Ability to recognise limitations of competence and seek appropriate advice.
• Experience working in General Practice.
• Experience of autonomous clinical assessment and decision-making.
• Experience managing acute and long-term conditions.
• Knowledge of EMIS.
• Experience leading service improvement or audits.

Qualifications

• Current NMC registration.
• Advanced Clinical Practice/Advanced Nurse Practitioner qualification or equivalent.
• Significant post-registration clinical experience.
• Experience with chronic disease management.
