Job description

Cannington Health Centre is seeking a highly skilled,autonomous Advanced Nurse Practitioner with independent prescribingqualifications to join our team. We are looking for an experienced practitionerwho can assess and manage acutely presenting patients within the practice. The successful candidate will be able to work independentlywithin the scope of their experience and qualifications, with support andsupervision available from the practices doctors.The post-holder must work within their professionalcompetence and adhere to the NMC Code of Conduct. Applicants must meet the NMCrequirements for advanced clinical practice.

Main duties of the job

Clinical - Assess, examine, diagnose, and develop management plans for patients presenting with a new problem. Deliver evidence-based care according to national and local guidelines.Prescribe within the scope of knowledge in accordance to both practice and National guidance. Produce accurate complete records of patient consultation ensuring easy and accurate coding enabling retrieval, monitoring and audit processes. Assisting to obtain the level of QoF points required by acting on highlighted QoF indicators where appropriate. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients in the application of the principles of self-care and healthy living. Follow agreed clinical protocols with referral to GPs as appropriate.

General - Take responsibility for your own learning and personal development. Have an awareness of and compliance with all relevant practice policies/guidelines, such as prescribing, safeguarding, confidentiality, data protection, health and safety. Communicate effectively with patients and carers. Carry out consultations within the practice face to face or by telephone. Undertake audits to evaluate the quality of the work within the practice and use results to implement improvements where required.

PRIMARYDUTIES AND AREAS OF RESPONSIBILITIES CLINICAL NURSING PRACTICE

The AdvancedNurse Practitioner works autonomously and is accountable for own professionalactions.

UndertakesClinical Nursing Practice at an advanced level using expert knowledge andclinical skills to deliver holistic care to people accessing primary healthcare services.

Assessesdiagnoses and treats patients attending primary health care services with arange of acute, non acute and chronic medical conditions.

Assesses diagnoses and treats patients insurgery or own home who require acute medical attention and refer toappropriate agency.

Directlyadmits patients to secondary care hospital in acute medical need. Referspatients to other medical specialities for assessment if deemed appropriate.

Referspatients as appropriate to other members of the multidisciplinary team,secondary care and to other statutory and voluntary organisations includingreferral for x-ray.

JOBDESCRIPTION - ADVANCED NURSE PRACTITIONER

Hasundertaken x-ray training as required by hospitals trust.

Interprets arange of diagnostic tests and routine clinical procedures.

Recognitionand involvement of Cannington Health Centre as a teaching unit and help drivethe Nursing element of training, including trainee NPs.
