Oxleas NHS Foundation Trust is seeking a committed, highly motivated Senior Staff Nurse to join the Rapid Response Team within Adult Physical Health. You will contribute to developing same-day urgent care pathways in the community alongside Social Care, Acute Trusts and Primary Care colleagues.

The Rapid Response Team is part of the SE London Accelerator Programme and delivers urgent health and social care assessments for adults 18+ in the Borough of Bexley, across hospital and community

Responsabilités

• Conduct comprehensive nursing assessments for adults in acute and community settings.
• Demonstrate sound clinical reasoning and risk assessment to guide appropriate nursing interventions.
• Work across hospital and community settings to respond to urgent referrals within the Rapid Response Team.
