Job summary

HCRG Care Group invites applications for a Community Matron in Sittingbourne, Kent. You will lead integrated care for patients at home, coordinating with multidisciplinary teams to improve outcomes and reduce hospital admissions.

Requirements include current NMC registration, substantial community nursing experience, autonomous practice and a full UK driving license. You will work in a supportive, award-winning service delivering high-quality, person-centred care across the locality.

Qualifications

• Current NMC registration.
• Significant post-registration experience in community/primary care or integrated care services.
• Experience managing patients with long-term conditions, frailty, and complex healthcare needs.

Responsibilities

• Manage a caseload of patients with complex health needs and frailty.
• Undertake comprehensive clinical assessments and develop personalised care plans.
• Coordinate and review care interventions for timely, effective, person-centred care.
• Support proactive case management and early intervention to reduce hospital admissions.
• Promote self-management, health promotion and preventative care.
• Recognise deterioration and exercise clinical decision-making.
• Provide clinical leadership and guidance within the team.
• Maintain accurate clinical documentation.
• Contribute to audit, quality improvement and service development.
