Job summary

Procare Community Services seeks a Community Matron to lead advanced clinical care for adults with complex long-term conditions, frailty, and multiple needs. You will coordinate integrated care with GPs, social care, and mental health teams to keep patients independent at home.

You will manage high-risk caseloads, review new referrals, and drive improvements in patient outcomes while working rotating late shifts and weekends as required.

Qualifications

  • Registered Level 1 Nurse with NMC registration.
  • MSc or level-7 qualification with health assessment module.
  • Experience in community/nurse-led management of complex cases.

Job description

Join Procare Community Services as a Community Matron and play a pivotal role in delivering proactive, person-centred care to adults with complex long-term conditions, frailty, and multiple health needs. Working within an Integrated Neighbourhood Team, you will provide advanced clinical assessment, case management, and leadership to support people to remain independent at home and reduce avoidable hospital admissions. You will work collaboratively with GPs, community services, social care, mental health teams, and other partners to coordinate holistic care and improve outcomes for patients and carers. Main duties of the job

The post holder will manage a caseload of individuals with complex needs and long-term conditions who are identified as being at high risk of unnecessary admission to hospital. You will need to have the nursing skills, experience, and expertise to assess, diagnose, treat, and evaluate health and needs.

All community nurses use EMIS as the clinical record system and are provided with a work Ipad and Iphone. We offer Agenda for Change Terms and Conditions and NHS Pension. As a Community Matron you will be required to work within the Community Coordination Centre on a rotational basis to cover late shifts and weekends, to clinically review and prioritise new, complex, and urgent referrals. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Detailed job description and main responsibilities

To provide expert clinical leadership and proactive case management for adults with complex and long-term health conditions, with a focus on preventing avoidable hospital admissions and supporting individuals to remain independent at home. Working as part of the Integrated Neighbourhood Team, the post holder will coordinate care across organisational boundaries, deliver comprehensive assessments, and develop personalised care plans that improve patient outcomes and experience.

Qualifications
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Qualification or experience equivalent to MSc / level 7, including health assessment module
Knowledge and Experience
  • Advanced clinical assessment and decision-making skills, including the recognition and management of deterioration, frailty, and long-term conditions.
  • Experience of managing complex caseloads and coordinating care for patients with multiple long-term conditions and complex health needs.
  • Knowledge of safeguarding, risk assessment, health and safety, and clinical governance principles.
