Job summary

Berkshire Healthcare NHS Foundation Trust is recruiting a Community Matron for the Bracknell area. You will join an integrated community nursing team to manage a complex caseload, applying advanced clinical skills and independent decision-making to deliver high-quality, person-centred care.

The post involves leading and coordinating care with the multidisciplinary team, promoting self-management and shared decision-making to improve health outcomes and reduce avoidable hospital admissions.

Qualifications

• Must be NMC registered and hold Non-Medical Prescribing qualification (Level 6/7).
• Extensive community nursing experience.
• Demonstrated advanced clinical assessment and decision-making skills.

Responsibilities

• Proactively manage a complex caseload of high-risk patients.
• Lead in coordinating care with the multidisciplinary team.
• Promote shared decision-making and self-management to improve outcomes.
• Support and develop junior colleagues within the community team.
