Job summary

HCRG Care Group seeks an experienced Community Matron in Sittingbourne to lead proactive, coordinated care for patients with complex health needs in their homes and community. The role sits within Integrated Care Teams, emphasizing autonomous clinical practice, care coordination and service development to reduce admissions and improve outcomes.

You'll work with multidisciplinary colleagues, provide leadership, ensure timely documentation, and support learning through our Learning Enterprise

Qualifications

  • Current NMC registration.
  • Significant post-registration experience within community, primary care or integrated care services.
  • Experience managing patients with long-term conditions, frailty and complex healthcare needs.
  • Strong clinical assessment and decision-making skills.
  • Full UK driving license and access to a vehicle.

Responsibilities

  • Manage a caseload of patients with complex health needs, long-term conditions and frailty.
  • Undertake comprehensive clinical assessments and develop personalized care plans.
  • Coordinate and review care interventions to ensure timely, person-centred care.
  • Support proactive case management and early intervention approaches.
  • Promote self-management, health promotion and preventative care strategies.
  • Recognise deterioration and respond with clinical decision-making.
  • Provide clinical leadership and support to colleagues within the Integrated Care Team.
  • Maintain accurate clinical documentation and contribute to quality initiatives.
  • Advocate for patients to keep their needs central to care planning.

About the Role

Are you an experienced community nurse looking to take the next step in your career, or an established Community Matron seeking a new challenge?We're excited to offer an opportunity for a Community Matron to join our Adult Community Services in Sittingbourne, Kent.Working within our Integrated Care Teams (ICTs), you'll play a pivotal role in delivering proactive, coordinated care to patients living with complex health needs, frailty and long-term conditions. You'll work closely with multidisciplinary colleagues and partner organizations to help people remain independent, improve health outcomes and reduce avoidable hospital admissions.This is a highly autonomous role that combines advanced clinical practice, care coordination, leadership and service development, enabling you to make a significant impact on the health and wellbeing of your local community.As this is a community-based position, a full UK driving license and access to a vehicle are essential.

Main Responsibilities
  • Manage a caseload of patients with complex health needs, long-term conditions and frailty
  • Undertake comprehensive clinical assessments and develop personalized care plans in collaboration with patients, carers and the wider multidisciplinary team
  • Coordinate and review care interventions to ensure patients receive timely, effective and person-centered care
  • Support proactive case management and early intervention approaches to reduce avoidable hospital admissions
  • Promote self-management, health promotion and preventative care strategies
  • Recognize and respond appropriately to signs of deterioration through effective clinical decision-making
  • Provide clinical leadership, professional guidance and support to colleagues within the Integrated Care Team
  • Maintain accurate and timely clinical documentation in accordance with professional, organizational and legal standards
  • Contribute to audit, quality improvement activities and service development initiatives
  • Act as an advocate for patients, ensuring their needs and preferences remain central to care planning and delivery
About You

We're looking for a compassionate and motivated registered nurse who is passionate about delivering outstanding community care and improving outcomes for patients with complex healthcare needs.

Essential Requirements
  • Current NMC registration
  • Significant post-registration experience within community, primary care or integrated care services
  • Experience managing patients with long-term conditions, frailty and complex healthcare needs
  • Strong clinical assessment and decision-making skills
  • Excellent communication, leadership and organizational abilities
  • Knowledge of health promotion, prevention and community-based care delivery
  • Ability to work autonomously while collaborating effectively within a multidisciplinary team
  • Full UK driving license and access to a vehicle
Desirable
  • Community Specialist Practitioner qualification or other relevant postgraduate study
  • Experience leading or contributing to service improvement and quality initiatives
  • Experience supporting workforce development, supervision or mentorship
  • A commitment to continuing professional development and advancing community care practice
