Nimbuscare is looking for experienced and enthusiastic clinicians to join our growing urgent care team. We are a clinically-led organisation committed to innovation, collaboration, and delivering care that puts people first.

Main duties of the job

As part of our team, you'll have the opportunity to work across our services, including:

  • Clinical Assessment Service (CAS), based in Holgate, York
  • Weekend Home Visiting Service based in York
  • Face-to-face sessions at York, Selby, Malton, Scarborough and Whitby Hospitals
  • Urgent Treatment Centres at York and Scarborough Hospitals
  • Same Day Urgent Care clinics in York, Scarborough and Whitby

We deliver Urgent Care services in collaboration with hospitals and GP practices across York, Scarborough, Whitby, Malton, and Selby. Our community-based frailty care focuses on treating people in the community, and we aim to shift services into the community where possible, such as our community diagnostic centres in partnership with the Acute Trust.

Job description

Job responsibilities

Clinical responsibilities:

  • Assess, diagnose, plan, implement, and evaluate treatment and care plans independently, ensuring safety, appropriate management, and timely referrals.
  • Work with the multidisciplinary team to facilitate triage, interventions, and reviews.
  • Prescribe independently (where permitted), review medications, and tailor treatments to patient needs following protocols and within scope.
  • Manage personal and team workloads effectively.
  • Provide clinical leadership aligned with safety frameworks and organisational policies.
  • Maintain effective communication within the team and with external stakeholders, including during admissions and discharges.
  • Support patients and families through difficult conversations, providing emotional support as needed.
  • Advocate for patients and colleagues, representing their needs and concerns.
  • Engage in quality improvement initiatives, responding to policies and assessing service impacts.
  • Maintain personal professional development and contribute to workforce strategy.
  • Support and mentor colleagues, including students and junior staff.
  • Assess ongoing care needs and plan transitions for continuity and quality.
  • Operate diagnostic equipment accurately and report faults.
  • Complete event reports when necessary.

Other responsibilities:

  • Ensure comprehensive, accurate, and up-to-date medical and patient records.
  • Complete coding forms with explanations of patient conditions and treatments.
  • Maintain timely documentation for referrals and care plans, including ReSPECT forms and advanced care planning.
  • Record all discussions regarding care appropriately and thoroughly.
  • Report events via the Nimbuscare system.
