Birmingham Community Healthcare NHS Foundation Trust is recruiting for a Band 8a post within a brand-new Single Point of Access service in Birmingham. The role supports urgent and community care, acting as a central hub for referrals, triage and advice.

You will join a multidisciplinary team, require NMC or HCPC registration and a MSc in advanced practice, and responsibilities include advanced clinical assessment, triage, and leading service improvements.

Qualificações

• Advanced clinical assessment of undiagnosed, undifferentiated illness.
• Experience in clinical triage and leadership within a multidisciplinary setting.
• CPD and ongoing professional development in line with advanced practice.

Responsabilidades

• Provide advanced clinical assessment as first contact for undiagnosed needs.
• Lead clinical triage, referrals and liaise with clinical teams.
• Coordinate referrals, review cases and manage pathways.
• Initiate investigations and interpret results within agreed pathways.
• Lead service to meet quality targets and high standards of care.
• Signpost and support access to services to reduce unnecessary A&E attendances.
• Supervise trainees and mentor staff within the service.
