Practice Plus Group — Health in Justice is seeking an Advanced Clinical Practitioner for the Primary Care team at HMP Altcourse. This full-time post focuses on assessing and managing complex patient problems and providing clinical leadership to the multidisciplinary mental health team.

The role involves leading on-site daily leadership input, ensuring patient safety, and delivering evidence-based, person-centred care aligned with CQC standards. Independent prescriber status is required.

Qualifications

• Independent or supplementary prescriber qualification.
• MSc in Advanced Practice.

Responsibilities

• Assess and manage complex patient problems with clinical judgement.
• Provide clinical leadership for the multi-disciplinary mental health team.
• Lead on-site daily leadership input with clinical guidance.
• Act as a clinical expert to ensure patient safety and up-to-date treatment.
• Deliver evidence-based care and personalise care to patient needs.
• Lead the team to provide safe, high-quality care meeting CQC standards.
• Promote best practice, clinical supervision and quality improvement.
• Share knowledge with clinical and non-clinical colleagues.
