Job summary

Oxleas NHS Foundation Trust is seeking an experienced Bank Advanced Clinical Practitioner – Primary Care to lead our primary care team within a challenging prison health setting.

The post holder will provide expert clinical leadership, empower colleagues, and ensure high standards of patient care across Mental Health, Physical Health, Pharmacy and Substance Misuse services integrated within the prison system.

Qualifications

• Registered nurse with primary care experience.
• Experience managing long-term conditions, QOF indicators and Section 7a.
• Must hold ACP or NMP qualification.

Responsibilities

• Lead the Primary Care team and support the Head of Healthcare.
• Provide quality primary care interventions to patients and carers.
• Lead integrated prison health care service and develop specialist clinical models.
