Lancashire & South Cumbria NHS Foundation Trust's Guild Lodge is seeking an experienced Advanced Clinical Practitioner to join the Physical Healthcare Team. The role combines clinical leadership with advanced assessment, diagnosis, investigations, and prescribing within scope to manage complex physical and mental health needs.

You will provide specialist care, develop evidence-based care plans, support long-term condition management, and contribute to clinical governance, audits, and service

Qualifications

• Candidate must hold MSc in Advanced Clinical Practice and a first degree in a registered health profession.
• Non-medical prescribing qualification required.
• Minimum five years post-qualification experience with leadership skills.

Responsabilités

• Provide specialist assessment and management of physical health conditions.
• Respond to acute physical health concerns within scope of practice.
• Support management of long-term conditions and preventative care.
• Undertake advanced clinical assessment, examination and diagnostic decision-making.
• Request and interpret investigations; prescribe within scope.
• Provide expert clinical advice to ward teams and multidisciplinary colleagues.
• Develop referral, escalation and clinical governance arrangements.
• Deliver clinical supervision, education and training.
• Lead audits, service evaluation and evidence-based improvement.
• Contribute to clinical pathways, guidelines and protocols.
