Job summary

James Street Group Practice is seeking an Advanced Clinical Practitioner (Mental Health) to deliver expert mental health care within a primary care setting. The role combines autonomous practice with collaboration in a multidisciplinary team to assess, diagnose, and manage complex mental health needs, aligned with NHSE Mental Health Curriculum and Capabilities Framework (2020).

The ANP will provide clinical leadership within the team, support GP colleagues, and contribute to service development

Qualifications

• Registered Nurse with current NMC registration.
• Masters-level qualification in Advanced Clinical Practice (Mental Health) or equivalent.
• Extensive post-registration experience in mental health, ideally in primary and secondary care.
• Competence across all four pillars of advanced practice: clinical, leadership, education, research.
• Excellent communication and interpersonal skills, autonomous working style, team collaboration.

Responsibilities

• Conduct comprehensive psychosocial assessments and risk formulation.
• Diagnose and manage a range of common mental health conditions.
• Develop and implement evidence-based care plans with patients, families and carers.
• Initiate, prescribe and monitor treatments in line with local formulary and NICE guidance.
• Screen for early indicators of disease, relapse, and risk factors.

Advanced Clinical Practitioner - Mental Health

To provideexpert mental health care within a primary care setting, working autonomouslyand collaboratively as part of a multidisciplinary team. The Mental Health ANP will assess, diagnose, and managepatients with common and complex mental health needswho wouldtraditionally be seen by the General Practitioner, contributing to improvedaccess, continuity, and quality of care. The role is underpinned by the fourpillars of advanced practice: clinical practice, leadership and management,education, and research.

The MentalHealth ANP will be able to demonstrate advanced level critical thinking in theclinical decision-making process. They will work with the practice team to meetthe needs of the service users and provide clinical professional leadership tothe team.

Main duties of the job

TheMental Health ANP will demonstrate an expert knowledge base, complexdecision-making skills, and competences for expanded scope of advanced clinicalpractice ensuring they are able to meet the competencies as set out in NHSEngland Mental Health Curriculum and Capabilities Framework (2020). Thespecific characteristics of the advanced practitioner role are shaped by theclinical context that the individual practices. They are part of themulti-professional team.

Job responsibilities

ClinicalPractice

Conduct comprehensive psychosocial assessments,including risk assessments and formulation. Making diagnostic decisionsbased on consultation, history, presentation & pathology.

Diagnose and manage a range of common mentalhealth conditions, including Depression, Generalised anxiety disorder(GAD), Panic disorder, Phobias, Obsessive compulsive disorder.

Develop and implement evidence-based careplans in partnership with patients, families, and carers.

Initiate,prescribe and monitoring the impact and outcome of treatments and medicationincluding efficacy, side effects, contraindications, and risk in line withlocal formulary and NICE Guidance.

Screenpatients for early signs of disease and risk factors and relapse indicators.

Identifyingand signposting concerns with physical illness.

Draw on adiverse range of knowledge in their decision-making to determine evidence basedtherapeutic interventions, which will include prescribing medication wherelegally allowed and actively monitoring the effectiveness of therapeuticinterventions.

Carry outwhere appropriate diagnostic and therapeutic procedures with due regard tomental capacity and advance decisions.

Plan andmanage complete episodes of care, working in partnership with others,delegating and referring as appropriate to optimise health outcomes andresource use, and provide direct support to patients and clients.

Refer toand liaise with secondary care services, substance misuse services, andvoluntary sector services as appropriate.

Order and interpret diagnostic tests& investigations.

Manage and proactively identify patientsat risk of relapse or deterioration, supporting their ongoing wellbeingand preventing crisis where possible.

Review patient annually on the SMIregister to ensure they receive appropriate care and treatment.

Maintain accurate, contemporaneous, andcomplete patient records consistent with legal and professionalguidelines.

Ensure dignity, privacy and cultural andreligious beliefs are always respected.

Adheres to professional code of conduct,works within the boundaries of their scope of practice and always manageassociated clinical risk effectively.
