Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust is seeking an experienced Advanced Clinical Practitioner to join the Stamford Unit, a 96-bed intermediate care facility. Responsibilities

• Work across four pillars of advanced practice with autonomous decision-making and leadership in assessing, diagnosing and managing patients with complex health needs.
• Provide advanced clinical care for rehabilitation, reablement, complex discharge planning and end-of-life care.
• Undertake comprehensive assessments, develop management plans, request and interpret investigations, and coordinate care to optimise outcomes.
• Collaborate with GPs, Therapists, Nurses, Social Workers and specialist teams to deliver safe, effective, person-centred care.
• Contribute to MDT discussions, treatment escalation and discharge decision-making.
• Support service development through quality improvement, audit, education and mentorship.
• Contribute to pathway development and promote continuous clinical excellence.
