Job summary

York And Scarborough Teaching Hospitals NHS Foundation Trust is seeking an Advanced Clinical Practitioner to lead patient-centred care in community inpatient units within York and Selby. The post holder will join a team of ACPs and a Trust Grade Doctor to deliver high-quality, coordinated care to patients closer to home.

Qualifications

• Registered practitioner (NMC/HCPC)
• MSc ACP or portfolio of practice accredited by the Centre for Advancing Practice
• Non-medical prescribing
• Resuscitation Council (UK) Immediate Life Support Provider
• Teaching and assessing qualification/mentor qualification
• IRMER certificate

Responsibilities

• Provide short-term rehabilitation, reablement, recovery and end of life care in community inpatient units.
• Practice as an Advanced Clinical Practitioner within the Community Inpatient Units as part of a team.
• Support admission from hospital and timely patient discharges from the in-patient units.
• Work across community teams to enable integrated, neighbourhood-based care.
