Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust is seeking GP leaders for Intermediate Care and Hospital at Home, with part-time hours and flexible practice arrangements. The Stamford Unit focuses on rehabilitation, complex discharge planning and step-up care for older adults, delivered in a patient-centered, multidisciplinary setting.

As a GP, you will oversee medical care, lead ward rounds and collaborate with ACPs, Nurses, Therapists and Social Workers to deliver safe, holistic

Qualifications

• Qualified GP with GMC registration and UK licence.
• CCT in General Practice.
• Full GMC Registration and National Performers List registration.
• Demonstrated ongoing professional development and leadership in primary care.

Responsibilities

• Provide senior clinical leadership across Intermediate Care and Hospital at Home services, seven days a week.
• Collaborate with senior managers to support Directorate objectives and ensure clinical engagement.
• Promote best practice and evidence-based care across the service.
• Drive continuous improvement to improve patient outcomes and pathways.
• Contribute to clinical governance and ensure robust processes are followed.
• Participate in audits, incident reviews and learning from complaints.
• Work flexibly to meet patient and service needs across settings.
• Engage with clinical networks to support system-wide learning and development.
• Support responsible use of resources and contribute to cost-improvement plans.
• Model Trust values and provide visible clinical leadership and professionalism.
