Job summary

HCRG Care Group is seeking an experienced Advanced Clinical Practitioner to join the Inpatient Unit team in Bath. You’ll provide advanced clinical assessment, diagnosis, treatment and management for inpatients, working autonomously and coordinating with consultants, nursing teams and therapists.

You will contribute to discharge planning, implement care plans, and support patient recovery in a hospital-at-home style service while maintaining high standards and ongoing professional development.

Qualifications

• Registered with NMC or HCPC.
• Accredited Advanced Clinical Practice qualification.
• Experience in frailty, elderly care or hospital at home.
• Proven experience managing complex caseloads.
• Leadership and supervision in multidisciplinary teams.
• Clean driving licence and access to a vehicle.
• Advanced Clinical Practice Digital Badge.
• Experience in Hospital at Home, Virtual Ward or Urgent Care services.

Responsibilities

• Provide advanced clinical assessment, diagnosis, and management for inpatient care.
• Work autonomously within scope of practice and support patient recovery.
• Discharge planning and liaison with GPs, relatives, and carers.
• Contribute to clinical audit, quality improvement, and service development.
