Job summary

HCRG Care Group is seeking an experienced Advanced Clinical Practitioner to join our Hospital at Home inpatient unit, based at Paulton Memorial Hospital with occasional cross-cover at St Martins Hospital. You will deliver advanced clinical assessment, diagnosis and management for inpatients and support safe, effective discharge planning.

The role requires autonomous practice, MDT collaboration with geriatricians and specialists, and a commitment to professional development and quality

Qualifications

  • Registered with NMC or HCPC
  • Accredited Advanced Clinical Practice qualification
  • Experience in frailty, elderly care, community services or acute care interfaces
  • Proven experience managing complex clinical caseloads
  • Leadership and supervision experience within multidisciplinary teams
  • Clean driving licence and access to a vehicle
  • Advanced Clinical Practice Digital Badge
  • Experience in Hospital at Home, Virtual Ward or Urgent Care services

Responsibilities

  • Provide advanced clinical assessment, diagnosis, treatment and management for inpatients requiring hospital-level care
  • Discharge planning and coordination with GPs, families and carers
  • Collaborate with consultants, geriatrics, nursing and therapy teams in MDT rounds and care planning
  • Contribute to clinical audits, quality improvement and service development

Job Introduction

We are looking for an experienced and motivated Advanced Clinical Practitioner (ACP) to join the HCRG Care Group Inpatient Unit team. This is an exciting opportunity to work within a supportive multidisciplinary environment, delivering high-quality, patient-centred care across our community inpatient services.

Based at Paulton Memorial Hospital, you will provide advanced clinical assessment, diagnosis, treatment and management for patients requiring inpatient care. Working autonomously within your scope of practice, you will play a key role in supporting patient recovery and facilitating safe, effective care.

Hours: 37.5 hours per week

Main Responsibilities

As an Advanced Clinical Practitioner , you will provide advanced clinical care to patients within our community inpatient rehabilitation services, primarily based at Paulton Memorial Hospital , with occasional cross-cover support at St Martins Hospital . You will assess newly admitted patients, formulate diagnoses and management plans, request and interpret investigations, and deliver high-quality, patient-centred care that supports rehabilitation and recovery.

Working autonomously within your scope of practice, you will manage complex clinical presentations and respond to acute health concerns that arise during a patient's inpatient stay. You will work closely with Consultant Geriatricians, specialist doctors, nursing teams, and therapists, contributing to ward rounds, multidisciplinary team meetings, and decision-making to ensure safe, effective, and coordinated care.

You will play a key role in discharge planning, referring patients to specialist services when appropriate and ensuring effective communication with GPs, relatives, and carers regarding significant changes in a patient's condition. You will also complete accurate discharge summaries, including medication information, treatment recommendations, and ongoing care plans to support a smooth transition back into the community.

In addition, you will uphold the highest standards of clinical practice while contributing to clinical audit, quality improvement initiatives, and service development projects. You will maintain your professional development through mandatory training, appraisal, and revalidation activities, joining a compassionate and forward-thinking team dedicated to achieving the best possible outcomes for patients in our local community.

The Ideal Candidate
  • Registered with the NMC or HCPC
  • Accredited Advanced Clinical Practice qualification
  • Significant experience in frailty, elderly care, community services or acute care interfaces
  • Proven experience managing complex clinical caseloads and working autonomously at an advanced level
  • Demonstrable leadership and supervision experience within multidisciplinary teams
