HCRG Care Group is seeking an experienced Advanced Clinical Practitioner to join our Inpatient Unit teams. The role delivers advanced clinical care within hospital-at-home/community inpatient services, collaborating with consultants, nursing and therapy staff to support rehabilitation and safe discharge planning.
You will assess patients, plan management, order investigations and participate in multidisciplinary rounds, audits, and service development.
As an Advanced Clinical Practitioner, you'll be part of our valued team in our Hospital at Home service based at in either Swindon or BaNES localities, you will provide acute hospital-level care to patients across the locality.
Working autonomously within your scope of practice, you will manage complex clinical presentations and respond to acute health concerns that arise during a patient's inpatient stay. You will work closely with Consultant Geriatricians, specialist doctors, nursing teams, and therapists, contributing to ward rounds, multidisciplinary team meetings, and decision-making to ensure safe, effective, and coordinated care. You will play a key role in discharge planning, referring patients to specialist services when appropriate and ensuring effective communication with GPs, relatives, and carers regarding significant changes in a patient's condition. You will also complete accurate discharge summaries, including medication information, treatment recommendations, and ongoing care plans to support a smooth transition back into the community. In addition, you will uphold the highest standards of clinical practice while contributing to clinical audit, quality improvement initiatives, and service development projects. You will maintain your professional development through mandatory training, appraisal, and revalidation activities, joining a compassionate and forward-thinking team dedicated to achieving the best possible outcomes for patients in our local community.
