Job summary

Lincolnshire Community Health Services NHS Trust is seeking an experienced Advanced Clinical Practitioner to join the Urgent Care Home Visiting Team. You will provide autonomous clinical assessment, diagnosis and treatment for patients with undiagnosed conditions, and contribute to service development.

The role offers protected time to develop your continuing professional portfolio, leadership and communication across the team, with 24/7 ACP shift patterns (0800-2030) seven days a week, and

Qualifications

MSc in Advanced Clinical Practice or HEE portfolio route to accreditation.
Non-medical prescribing qualification where appropriate.
Portfolio evidence across the four Pillars of Advanced Practice.

Responsibilities

Autonomous practitioner with independent clinical assessment, diagnosis and treatment.
Develop audit and research projects within the team.
Provide ongoing supervision and support to medical and non-medical colleagues.
Contribute to operational development of the specialty and maintain CPD portfolio.
