Job summary

Cardiff and Vale University Health Board is seeking an experienced Advanced Clinical Practitioner to provide autonomous care within the Urgent Care Home Visiting Team. The post involves independent clinical assessment, diagnosis and treatment of undiagnosed conditions as part of a multidisciplinary team.

You will contribute to audit and research projects, provide ongoing supervision and support to medical and non-medical colleagues, and play a pivotal role in service development.

Qualifications

  • Registered Health Care Professional with current registration
  • MSc in Advanced Clinical Practice or evidence for HEE portfolio route accreditation
  • Non-Medical Prescribing qualification
  • Teaching/Assessing course or willingness to complete (e.g., Mentor Preparation, SLIP, Clinical Educator)

Job description

  • Registered Health Care Professional holding current registration with appropriate regulatory body
  • MSc in Advanced Clinical Practice or sufficient evidence to support an application onto the HEE supported portfolio route for accreditation
  • Non-Medical Prescribing qualification
  • Teaching and Assessing course (e.g. Mentor Preparation, SLIP, Clinical Educator course, PGCE), SSSA or a willingness to complete.

Successful candidates will need to demonstrate that they are able to meet the essential criteria in the person specification and can provide evidence of significant experience in their specialist field. You will be required to have and an MSc in Advanced Clinical Practice or have completed the Health Education England supported portfolio route to accreditation. Candidates will also hold a non-medical prescribing qualification where appropriate.

We are looking for people who can demonstrate the right values and behaviours and have excellent leadership and communications skills. Portfolio evidence of core capability across all four Pillars of Advanced Practice is expected. As part of a team, you will work as an autonomous practitioner, being accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. The role will include developing audit and research projects, providing ongoing supervision and support to medical and non-medical colleagues, and play a pivotal role in the operational development of their specialty. The post holder can expected protected time to fulfil these duties and maintain their continuing professional development portfolio. This post is within our Urgent Care Home Visiting Team which support patients that require a urgent health care need. The service is 24/7, with ACP shift patterns of 0800-2030, 7 days a week.,

  • Substantial post registration relevant experience working autonomously at an advanced level and can demonstrate transferrable skills.
  • Experience of clinical leadership.
  • Experience of working within multi professional settings with consolidated clinical practice and contributing to effective team working.
  • Experience of leading evidenced service improvement and innovation through service/practice development initiatives, audit or research., Completed portfolio of supervised practice in specialist area
