Job summary

Cardiff and Vale University Health Board is seeking Advanced Clinical Practitioners / ANPs to join our bank workforce with opportunities within Community Services based within the Asylum Service.

You will assess, diagnose and plan care for patients with undifferentiated or acute health needs, working autonomously in urgent and primary care settings to deliver high‑quality NHS care. NMC registration is required; MSc in ACP is desirable.

Qualifications

• NMC registration with no restrictions.
• Experience in urgent care or primary care.
• Ability to assess, diagnose and plan patient care.
• Working towards MSc in Advanced Clinical Practice or equivalent.

Responsibilities

• Assess, diagnose, plan, implement and evaluate treatment and care for patients with undifferentiated, acute, complex, or vulnerability-related health needs.
• Clinically examine and assess patient needs from physiological and psychological perspectives and plan care accordingly.
• Identify, diagnose and manage treatment plans for patients at risk of long-term conditions.
• Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatments.
• Ensure referrals for health, social care or safeguarding needs are escalated through the appropriate pathway within the required timeframe.

Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs/ ANPs to join our bank workforce with opportunities within our Community Services based within the Asylum Service.

• Assess, diagnose, plan, implement and evaluate treatment, interventions and care for people presenting with undifferentiated, undiagnosed, acute, minor, complex or vulnerability-related health needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.
• Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatment methods into a management plan.
• Ensure any patient requiring referral for an identified health, social care or safeguarding need is referred or escalated through the appropriate pathway within the required timeframe., Full NMC Registration with no restrictions
• MSc in Advanced Clinical Practice is desirable; candidate who are working towards this qualification or who have relevant experience are encouraged to apply
