A leading healthcare practice in Herne Bay seeks an Advanced Practitioner to join their dynamic team. This role involves managing patient care with a focus on minor illnesses, providing both face-to-face and remote consultations. Park Surgery is looking for a qualified AdvancedPractitioner to join our innovative and forward thinking Practice helping us toprovide healthcare to approx. 24,500 patients in Herne Bay.
Salary is in a range equivalent to Agenda for Change bands7-8a depending on experience and qualifications. We offer 6 weeks annual leave,and there is scope for additional training and development.
This is a role funded under the Additional RolesReimbursement Scheme. Applicants will need to meet the requirements for theAdvanced Practitioner role under the PCN DES.
You will be an Advanced Clinical/Nurse Practitioner and preferablyan Independent Prescriber and able to manage patients autonomously. These willbe with a minor illness focus across the age range.
You will undertake a variety of duties including face toface consultations, telephone consultations, and home visits. This will be forpatients across the age range.
You will be responsible for providing NHS medical servicesto patients on the Practice list. This will have a minor illness focus but canbe shaped based on particular clinical knowledge and experience.
We are a dynamic, enthusiastic and well regarded Practicelocated in the beautiful seaside town of Herne Bay in East Kent.
We have an extensive clinical team made up of GPs, AdvancedClinical Practitioners (Paramedics and Nurses), Specialist Nurses and HCA's. Wealso have a First Contact Physiotherapist, Pharmacists, Pharmacy Technician andSocial Prescribers together with a large administrative team.
As a training Practice we are passionate about theeducation and training of all health professionals and provide regulartutorials for our ACP team
We have a great relationship with our neighbouring Practicehaving worked well with them for many years developing new health services forthe population of Herne Bay. We work closely as part of an excellent PrimaryCare Network (PCN) to provide first class care to our patients, including anUrgent Treatment Centre, Ophthalmology, MSK and ENT services, and if you wantto develop special interests in the future, we would be happy to discuss howthis might fit in with the PCN and practice plans.
We have a large elderly population and provide regular wardrounds for our care homes as part of the local PCN.
The AP will work to deliver high quality clinical care bothin the practice and on home visits. The split is flexible and can be agreed tomeet the preference of the AP and the needs of the practice. The role willinvolve assessment, diagnosis and treatment planning of patients across the agerange with support from advanced practitioners and GPs.
Park Surgery is committed to the education and developmentof its staff, and APs will be supported to accredit with Health EducationEngland when this becomes available for their profession.
The position is open to nurses or paramedics meeting theAdvanced Practice Four Pillars in Primary Care Medicine.
