Job summary

Ash Grove Medical Centre is seeking an experienced Advanced Clinical Practitioner (ACP) to join our friendly, forward-thinking team. You will deliver autonomous clinical care across primary care, working closely with GPs and the wider multidisciplinary team.

Your duties include seeing patients in clinic, undertaking home and care home visits, reviewing investigations, and developing management plans. You will provide treatment, follow-up, and ensure safe, effective care while supporting service

Job description

As an ACP, you will provide autonomous clinical care across our general practice, working closely with our GPs and wider multidisciplinary team.

Your role will include seeing patients in clinic, undertaking home and care home visits, assessing and managing a varied range of acute and chronic presentations and reviewing clinical investigations and pathology. You may also see children where appropriately qualified and experienced.

You will use your advanced clinical skills and professional judgement to assess patients, develop appropriate management plans, provide treatment and follow up and ensure patients receive safe and effective care.

The role is varied and offers the opportunity to work across different areas of primary care while contributing to the development of our clinical services and supporting the wider practice team.
