Job summary

Brewood Medical Practice is seeking an Advanced Clinical Practitioner to join the clinical team. The post involves autonomous assessment, diagnosis and treatment of patients across surgeries, phone consultations and home visits, four days a week with prescribing for chronic disease.

Applicants must hold a Master’s or equivalent qualification covering clinical practice, leadership and education, plus a prescribing qualification.

Responsibilities

• Assess, diagnose and treat patients with acute, non-acute and chronic conditions, making direct referrals where appropriate.
• Work autonomously and be accountable for own professional actions, alongside other healthcare professionals.
• Manage daily surgeries of face-to-face and telephone consultations, house visits and telephone triage.
• Deliver holistic advanced-level clinical care to people accessing primary care.
• Carry out home visits for chronic and acutely unwell patients, including certain medical emergencies.
• Prescribe medication for areas of chronic disease.

Job description

This Advanced Clinical Practitioner post sits in the clinical team at Brewood Medical Practice, a semi-rural General Practice with around 10,700 patients. The role centres on working autonomously to assess, diagnose and treat patients with acute, non-acute and chronic conditions across surgeries, telephone work and home visits. It is offered for four days a week and includes prescribing for chronic disease. A master's-level qualification and a prescribing qualification are required.
