Cheshire and Wirral Partnership NHS Foundation Trust is seeking an Advanced Clinical Lead for the inpatient dementia assessment team on Silk Ward. You will lead clinical practice, drive quality improvement and support a multidisciplinary team to deliver high standards of patient care in line with trust guidelines.
The role emphasises autonomous leadership, expert assessment of complex cases, staff development and shaping a culture of learning, collaboration and compassionate care across services.
An exciting opportunity has arisen to join a well-established inpatient specialist dementia assessment team on Silk Ward, in the role of an Advanced Clinical Lead.
demonstrate highly effective communication skills and problem-solving abilities, implement continuous quality improvement initiatives with current best practice for dementia care, identifying learning themes and responding to this, attendance at Trust wide meetings and local Dementia Steering Group, monitoring and evaluating the effectiveness of clinical standards.
working autonomously within a multi-disciplinary framework and act as a source of expertise and point of contact within the team, supporting with complex cases with specialist assessment and planning of care, liaising with internal and external professionals and working collaboratively, and ensuring high standards of patient care within trust guidelines and policies.
delivery of training and supporting with sourcing further training/education opportunities, facilitating clinical supervision, providing a coaching and mentoring role to enhance the teams skills and knowledge, motivating and inspiring the team to drive high standards of patient care and creating a productive work environment.
