Job summary

Sanctuary Personnel is seeking an experienced Adults Social Worker to join the Preparing for Adulthood Team in Rotherham, South Yorkshire. This locum, ongoing role offers an hourly rate of £33.12 and the opportunity to support young people transitioning into adulthood.

You will manage caseloads, undertake assessments and care planning, collaborate with families and multi-agency partners, and ensure safeguarding.

Qualifications

• Hold a recognised Social Work qualification and be registered with Social Work England.
• Minimum of two years’ post-qualification experience in social work practice.
• Experience of using LAS (Liquidlogic Adult System).
• Sound understanding of Care Act 2014 and Mental Capacity Act 2005.

Responsibilities

• Manage a caseload of young people transitioning into adulthood, providing essential guidance and support.
• Undertake assessments, reviews, and care planning in accordance with legislative and organisational requirements.
• Collaborate with individuals, families, carers, and multi-agency professionals to develop holistic support plans.
• Champion independence, choice, and control through strengths-based practice.
• Safeguard vulnerable adults, ensuring risks are appropriately assessed and managed.
• Maintain accurate and timely case recordings and documentation.

To shine in this role, you must:
