Job Description:

Key Duties/Areas of Responsibility

• Provide a local and personal social care service for vulnerable customers and contribute to the range of voluntary and statutory social work activities.
• Carry out investigations and planned assessments, providing written evidence-based reports for highly complex cases.
• Assess customer needs in accordance with Council policy and relevant legislation, including joint assessments with other bodies, developing preventative intervention strategies and support services.
• Develop support plans, identify, negotiate, and coordinate necessary services, utilizing informal support networks where possible, and authorize care packages within financial constraints and delegated powers.
• Monitor support plans to ensure service delivery meets standards and review individual circumstances, amending services as needed.
• Promote principles of prevention, reduction, and delay, ensuring support recipients' rights to dignity, respect, and self-determination are upheld.
• Provide information, advice, and guidance to customers and carers on local services and support options from voluntary and statutory agencies.
• Offer ongoing advice and support to vulnerable clients and their carers, including case management roles.
• Maintain accurate records and reports using the department's IT systems.
• Liaise with other organizations to develop a multi-agency approach for service continuity.
• Participate in developing preventative intervention strategies and support services.

Must be registered with Social Work England

• Undertake safeguarding investigations as appropriate.
