Job summary

Medical Update Online in Bridgend is seeking an experienced Adults Social Worker to join the Integrated Network Team on an initial 3‑month contract. The role is full‑time with hybrid working (2 days at home, 3 days on site) and involves working in a multidisciplinary context to support adults 18+ with frailty and long-term conditions.

You will assess and plan care, coordinate with partners, and visit clients regularly.

Qualifications

• Qualified Social Worker with post-qualifying adults experience.
• Registered with Social Care Wales.
• Confident working within an MDT environment.
• Strong assessment, care planning and care coordination skills.
• Able to work autonomously while contributing to a wider team.

Responsibilities

• Adults community social work within an Integrated Network Team.
• Managing complex cases with a strengths-based, person-centred approach.
• Care coordination and multi-agency working.
• Regular visits required (driver essential).
