Job summary

Somerset Council is seeking a candidate to join the Transition to Adult Social Care team, assisting young people and families as they move from Children’s to Adult Social Care. You will assess care needs, plan with families, and coordinate access to community and health services, with strong emphasis on rights and independence.

You will build trust, communicate clearly, and work alongside Social Workers, health professionals and education colleagues.

Qualifications

• Experience in a caring or supportive role, paid, voluntary or similar setting.
• Ability to advocate for individuals and carers and uphold rights.
• Empathy, respect and ability to build trust with diverse people.
• Confidence to work independently and seek guidance when needed.
• Strong organisational skills and accurate record-keeping using IT systems.

Responsibilities

• Complete strengths-based assessments and reviews with young people and carers.
• Co-create personalised, outcome-focused support plans promoting independence and choice.
• Provide information and support about community services, housing, benefits, education and employment.
• Attend transition planning meetings and multi-disciplinary forums.
• Work with Social Workers, Occupational Therapists and health partners.
• Recognise safeguarding concerns and escalate as appropriate.
• Coordinate specialist input or advocacy when needed.
• Arrange, monitor and review commissioned services and self-managed support.
• Manage an agreed caseload with autonomy and prioritisation.
• Maintain case records and contribute to practice audits and supervision.
