Job summary

Rotherham Metropolitan Borough Council is seeking an Assessment & Review Co-Ordinator (PFA) to support young adults as they prepare for adulthood. The role is permanent, full-time, 37 hours per week and based in Rotherham.

You will work within the Preparing for Adulthood team, collaborating with partner agencies to promote independence and wellbeing, and you will apply statutory frameworks to safeguard and support people in transition.

Qualifications

• Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, Making Safeguarding Personal, and Continuing Healthcare Frameworks.

Responsibilities

• Work with young adults to prepare for adulthood.
• Collaborate with partner agencies to maximise independence and resilience.
• Support people in crisis to ensure timely access to services.
