Job summary

Somerset Council is seeking an Adults Social Worker to join the Learning Disabilities team. You will undertake detailed Care Act reviews, carry out face-to-face visits and gather evidence to assess care and support needs.

The role requires professional judgment, collaboration with health colleagues and providers, and the ability to produce audit-ready assessments and plans. Travel to Somerset sites is expected.

Responsibilities

• Undertake detailed Care Act reviews and reassessments for adults with learning disabilities.
• Complete face-to-face visits in homes, supported living, residential settings and community environments.
• Gather, analyse and evaluate information from multiple sources to inform decisions.
• Record clear professional rationale for decisions and determine if a full reassessment is required.
• Identify, evidence and record eligible care and support needs under the Care Act.
• Produce high-quality, evidence-based assessments, reviews and care plans.
• Collaborate with Adult Social Care teams, health colleagues and providers.
• Attend Somerset office locations (minimum two days per week) and travel across Somerset for visits and assessments.
