Job summary

Wirral Council is seeking a dedicated Social Worker to join our team, delivering personalised, strengths-based support for adults and carers across the borough. The role is split between Clatterbridge Hospital and Arrowe Park Hospital/Clatterbridge Hospital, offering development opportunities within a dynamic multidisciplinary team.

You will work to Care Act principles, assess complex needs, support safeguarding, and help with timely discharges while promoting independence and positive outcomes.

Qualifications

• Experience with assessments, safeguarding, and care planning.
• Ability to work within statutory frameworks and Care Act 2014.
• Strong written and verbal communication.
• Experience working with adults with complex needs and carers.

Responsibilities

• Deliver personalised, strengths-based support for adults and carers.
• Support safe, timely discharges from hospital with D2A pathways.
• Contribute to high-quality health and social care delivery.
• Model best practice and support ASYE/newly qualified workers and Care Navigators.
• Promote independence, wellbeing and positive outcomes.
